Structured Family Caregiving (SFC) is a program in Georgia that provides an alternative to nursing home care, allowing a person to remain at home while a family member or close friend serves as the paid primary caregiver. It supports the caregiver with a monthly stipend, a dedicated care team, and training, so loved ones stay where they're most comfortable.

Medicaid eligibility

The care recipient must be enrolled in (or eligible for) Georgia Medicaid under a CCSP or SOURCE waiver.

Functional / medical need

The care recipient has a functional disability or medical condition requiring daily assistance with activities of daily living (bathing, dressing, mobility, etc.).

Family caregiver requirement

The caregiver is a family member or close friend, 18 or older, and able to provide the needed care.

Residency & employment

The caregiver lives in the same home as the care recipient and provides care as their primary focus (not working a separate full-time job that prevents caregiving).

Relationship of caregiver

The caregiver may not be the spouse or legal guardian of the care recipient (Georgia Medicaid rule).

Background check

The caregiver must pass a background screening.
